Perimenopausal Hair Loss
Perimenopausal hair loss is something many women don’t expect—but it’s extremely common. It typically begins around the late 40s (average onset ~49), and while it can resemble Androgenetic alopecia, that condition more often starts earlier, usually before age 40.
This patient, in her late 50s, came in with a several-year history of gradual thinning—especially along the crown and part line. Like many women in this stage of life, she was concerned not just about hair loss, but about how quickly it seemed to progress once it started.
On exam, there were no signs of scarring or inflammation. There was widening of the midline part especially on the frontal scalp. Based on the clinical picture, we diagnosed perimenopausal alopecia, a hormonally driven form of hair thinning that overlaps with female pattern hair loss but is often under-recognized.
