For your convenience, we have supplied forms which can be printed out. To save time, you can fill out these forms at home and then bring them with you to your appointment. We look forward to greeting you!

Prescription Refills

To refill your prescription, contact your pharmacist before calling Aglow Dermatology, and ask if you still have one or more refills. If it turns out that you are out of refills please read our Pharmacy/Refill Policy found below.

If you haven’t seen Dr. Strachan for more than six months, you need to come in for an appointment before we will approve a prescription refill. This is because we need to check that the medications are working properly for you and aren’t causing side effects or other problems of which you may or may not be aware. Additionally, there may be a newer or more effective therapy available for your condition.

Insurance Authorization

Some insurance plans require a patient to get prior authorization or a referral before he or she can see a specialist such as a dermatologist. You can find out if you need a referral by calling your insurance company. Sometimes this information is on your insurance card. It is the patient’s responsibility to obtain a referral from the primary care physician and present it at the time of the appointment. Unfortunately, if the referral is not available at the time of visit, the appointment will have to be rescheduled.

Payments

As you know, there are charges for each of the medical, cosmetic and aesthetician services we provide. Payments for office visits and procedures are due at the time of service. As a courtesy, we participate in a number of health insurance plans for the purposes of providing your medical care. Insured patients may have additional financial obligations such as co-payments, deductibles and coinsurance. The amounts of these patient responsibilities are determined by your insurance contract and we are required by law to collect them. Co-payments are due at the time of service. If there are deductibles or coinsurance, you will be billed at a later time. We accept cash, credit card, or check. Payments for balances due may be paid by mail, over the telephone, or online.

Payment Methods

Any payments required on the day of your appointment may be made by cash, check, or credit card. We accept MasterCard, Visa, American Express and Discover. Financing of medical and cosmetic services is also available through CareCredit. Payments for bills sent after the appointment can be paid by these methods, by telephone, or online.

Courtesy Appointment Reminders

We offer courtesy appointment reminders via e-mail, text message, and voice. Patients have been very pleased with the convenience of confirming or canceling with a press of a button! In addition, we offer an electronic newsletter, and you can read our Blog for dermatological topics of interest and other practice updates. Please contact us with your current mobile phone and e-mail address in order to take advantage of these options.
