To save time, you can fill out these forms at home and then bring them with you to your appointment.

Prescription Refills

If you need a refill on your prescription, contact your pharmacy before calling Dr. Strachan, since you may still have one or more refills. If it turns out that you are out of refills, have the pharmacy contact us. We can usually approve refills within five business days.

If you haven’t seen Dr. Strachan for more than six months, you will need to come in for an appointment before we will approve a prescription refill. We do not refill prescriptions for patients who do not keep appointments. This is because we need to make sure the medications are working properly for you and aren’t causing side effects or other problems of which you may or may not be aware. Additionally, there may be a newer or more effective therapy available for your condition.

Insurance Authorization

Please be advised that it is the patient’s responsibility to obtain the necessary authorizations from their primary care physician or insurance company prior to seeing Dr. Strachan. These authorizations vary with each insurance company, and we advise that you consult your insurance company or your primary care provider to determine if any pre-authorization or doctor referrals are required. If your insurance company requires an authorization, this authorization form or referral must be provided at the time of your visit to the doctor.

Payment Method

Payments required on the day of your office visit may be made by cash, check, or credit card. We accept all major credit cards and CareCredit.

Payments and Deductibles

All payments for office visits, if required by your medical insurance company, are due on the day of your appointment with the doctor. Any co-payments required by your insurance plan will be expected to be paid on the day of service, and for patients seeing our doctor out-of-network, the full office visit fee is payable at the time of the appointment. Balances are due once claims are processed by insurance companies and exact amounts owed by patients are determined. We accept payments for balances via cash, credit card, or check, by mail or via telephone.

Courtesy Appointment Reminders

We now offer appointment courtesy reminders via e-mail or text message. Patients have been very pleased with the convenience of confirming or canceling with a press of a button! In addition, we offer an electronic newsletter, and you may also follow our Blog for dermatological topics of interest. Please contact us with your current mobile phone and e-mail address.
